Botox Subscription Terms Of Service

All Botox Treatments must be at least 60 Days apart. Up to four treatments allowed per year with your subscription.  Additional treatments may be purchased at our regular pricing.  Additional units may be purchase at the discounted price of $11/unit at any of your four treatments. 

You may cancel your subscription at any time.  However, if you cancel your subscription prior to one year you will be charged a cancellation fee of $100 plus the full price of any Botox treatments already received, minus all payments made. Subscriptions canceled after one year will waive the $100 cancellation fee.

These prices are already lower than annual membership pricing so membership discount does not apply.

Payments will be automatically charged to your credit card on file each month.
